Allure Salon COVID-19 Release Form & Waiver

The undersigned Client agrees with Allure Salon that no services will be performed for Client without this document being executed by Client and delivered to Allure Salon.

Client agrees to have services performed for Client by employees of Allure Salon during the COVID-19 pandemic and assumes the risks of doing so. I understand and acknowledge that due to the frequency of visits of other clients, the characteristics of the virus, and the characteristics of hair services, that despite the efforts of all parties, Client has an elevated risk of contracting the COVID-19 simply by being in the salon.

Client understands the COVID-19 virus has a long incubation period during which carriers of the virus may not show symptoms and still be highly contagious, and that it is impossible to determine who has the virus and who does not, given the current limits in virus testing.

Client confirms that Client is not presenting and has not presented in the last 14 days any of the following symptoms of COVID-19:

Client by signing below verifies that Client has not traveled outside the United States in the past 14 days. Client further verifies that Client has or has not traveled domestically by commercial airline within the past 14 days.

To help prevent the spread of COVID-19 and to help protect each other, Client agrees that Client will have to follow the salon’s strict guidelines. Client understands that the CDC, OSHA, and Washington State Board of Cosmetology recommend social distancing of at least 6 feet.

Client, by signing below and accepting services from Allure Salon, hereby releases Allure Salon from any liability and waives any claims against Allure Salon and any of its owners or employees for damages or injury arising from or related to COVID-19, and acknowledges that Client is assuming all risks of contracting COVID-19 by accepting services from Allure Salon.
